Credentialed Pharmacist, B.Pharm (Hon), FANZCAP (GeriMed, MedsMgmt), BPS Geriatric Pharmacist.
Vybol is a Board‑Certified Geriatric Pharmacist with 20 years of experience across
community and hospital pharmacy settings. He currently practises as a credentialled
pharmacist specialising in Home Medicines Reviews conducting medication reviews at the
convenience at the patient’s home.
Vybol is committed to evidence‑based, patient‑centred care and dedicated to enhancing
medication safety and therapeutic outcomes for older adults. He works collaboratively with
GPs, specialists, and allied health professionals to support high‑quality, integrated, and
preventative care across the continuum of ageing.
Mrs. Ly Sith
Credentialed Pharmacist, B.Pharm (Hon), FANZCAP (GeriMed, MedsMgmt), BPS Geriatric Pharmacist.
Ly is an experienced pharmacist with two decades of practice supporting older adults to use
their medicines safely and confidently. She has worked across a wide range of community
and hospital settings and brings a practical, patient‑focused approach to medication
management.
As a credentialled pharmacist, Ly provides Home Medicines Reviews (HMRs), visiting
patients in their homes to offer personalised support with their medicines. During an HMR, she helps patients understand what each medicine is for, checks for any medication‑related issues, and works closely with their GP to ensure every treatment is safe, effective, and tailored to their needs.
Her focus is on making medicines easier, safer, and more manageable for older adults, while
supporting their GP with clear, evidence‑based recommendations.
